ABSTRACT

Pegylated interferon and ribavirin combination therapy is accepted as the standard antiviral treatment for chronic hepatitis C regardless of HCV genotype. This combination therapy achieves higher response rates than previous therapy, but, nevertheless, a large proportion of patients suffer from treatment failure or adverse events. Recent clinical studies of viral kinetics during antiviral treatment have led to the introduction of response-guided therapy, the concept of 'customized therapy depending on viral response', which focuses on modulation of the treatment period depending on the viral response to create a sustained viral response without unnecessary medication and costs. New upcoming direct-acting antivirals (DAAs) maximize response rate, and triple therapy including DAAs along with pegylated interferon and ribavirin combination therapy could soon be the standard therapy. In this article, we reviewed the factors affecting treatment, response guided treatment, retreatment after failure of standard treatment, management of adverse events during treatment, and new treatment options.

ABSTRACT

Dogs and cats are the animals that owners most frequently seek assistance for potential poisonings, and these species are frequently involved with toxicoses due to ingestion of poisonous food. Feeding human foodstuff to pets may prove itself dangerous for their health, similarly to what is observed in Allium species toxicosis. Allium species toxicosis is reported worldwide in several animal species, and the toxic principles present in them causes the transformation of hemoglobin into methemoglobin, consequently resulting in hemolytic anemia with Heinz body formation. The aim of this review is to analyze the clinicopathologic aspects and therapeutic approach of this serious toxicosis of dogs and cats in order to give knowledge to veterinarians about Allium species toxicosis, and subsequently allow them to correctly diagnose this disease when facing it; and to educate pet owners to not feed their animals with Allium-containg food in order to better control this particular life-threatening toxicosis.

ABSTRACT

Se describe el desarrollo de Leucemia Mielomonocítica Aguda (M) en una paciente de 76 años tratada con Azatioprima por Anemia Hemolítica Autoinmune refractaria a esteroides. Se señalan las alternativas hematológicas producidas por la Azatioprima y los posibles mecanismos para la leucemogénesis. Debido a que con mayor frecuencia las enfermedades no neoplásicas tratadas con inmunosupresores que han terminado en leucemias agudas, no incluyen la anemia hemolítica autoinmune, se ha considerado importante la descripción de esta asociación

ABSTRACT

A descriçäo da anemia hemolítica em cirurgias reparadoras da valva mitral é pouco relatada na literatura. No presente trabalho apresenta-se esta complicaçäo sob o ponto de vista de critérios diagnósticos e aspectos clínico-cirúrgicos, associada à cerclagem do anel posterior da valva mitral com auxílio de tiras de teflon. Este fato ocorreu em duas crianças sendo uma delas tratada, clinicamente, com ácido fólico e suprimentos de ferro. A outra criança necessitou de nova cirurgia para a substituiçäo da tira de teflon por outra de pericárdio bovino e ampliaçäo do folheto posterior da valva mitral


The description of hemolytic anemia after mitral valve repair has been infrequent. The present paper presents this complication, discussing its diagnosis and surgical aspects, such as the association with the use of teflon felt for posterior annuloplasty. This complication happened in two child ren at our institution, one treated medically with suplementation of folic acid and ferrous sulfate and another requiring surgery for replacement of the teflon felt using a piece of heterologous pericardium for a new annuloplasty and leaflet advancement..
